Optical Benefit for Active Members

Davis Vision is the exclusive provider for active DEA members and their dependents.

The Davis Vision plan provides DEA active members with uniformity and a wide selection of frames from the Exclusive Collection. The Exclusive Collection is available at independent provider locations. To find a location, visit www.davisvision.com and click on “Find A Provider.”

Members have the opportunity to select any frame from the Fashion, Designer, and Premier level of frames from the Davis Vision Exclusive Collection. The DEA’s goal is to deliver a plan that drastically reduces (or eliminates altogether) the high member out of pocket costs associated with current state of the art eyewear, as well as comprehensive eye examinations. Second and third pairs are available at applicable copayments. The copayment is $65 for each pair and the frames from the Fashion and Designer Collection can be chosen.

The plan offers a comprehensive eye examination and a pair of glasses to members and their dependents once every 12 months. In lieu of eyeglasses, you may select contact lenses. Any contact lenses from Davis Vision’s Contact Lens Collection will be covered in full per the number indicated below, and your evaluation, fitting, and follow up care will also be covered. Davis Vision Contact Lens Collection (includes evaluation, fitting, and follow-up):

Disposable == Eight boxes/multi-packs

Planned Replacement == Two boxes/multi-packs

In lieu of the Davis Vision contact lenses, members may use their $60 credit to go toward the provider’s own supply of contact lenses, evaluation, fitting, and follow-up care. This credit would also apply towards all contact lenses received at participating retail locations.

In order to provide additional convenience for our members, eligibility will be determined electronically, thereby eliminating the need for DEA-issued vouchers. Simply provide the participating Davis Vision provider with your unique DEA identification number located on your DEA Vision Benefit Card. The plan also features a one-year breakage warranty, and discounts on Lasik and hearing aids.
