There is another important update on the Department’s COVID-19 testing program. As the Department has now informed us, unvaccinated members are being directed to receive a COV1D-19 test at one of five Department-run testing sites:

These COVID testing sites will be open Monday to Friday from 06:00 to 18:00 hours.
Quest Diagnostics Labs will conduct the testing. Prior to testing, MOS must set up a Quest account using their insurance information at —

At least one-hour prior to taking the COVID test, the MOS must log into the above website to request the test. Once registered, the MOS can report to any of the above testing sites. In addition, each MOS will be required to fill out a short requisition form. This form will be sent out via Department email and should be completed prior to arrival at the testing site. Quest Diagnostics will provide lab results via the MyQuest account. COVID-19 test results must then be uploaded to the centralized personnel resource (CPR) system profile each week.
Members must bring their Department ID and insurance card. There is no copay and members will not be billed.

As per our discussions with the Department, members are being directed to these sites on job time and with no cost to the member. Members are advised that receiving a test somewhere other than these five locations could result in out-of-pocket costs.
The Department has also said that they may expand the number of testing sites, and they are still planning to deploy self-administered testing kits to commands without a testing site as soon as those kits are made available by the City. Until then, members should avoid incurring out-of-pocket costs by only receiving tests from a Department-run site or a self-administered kit issued at their commands.
As we stated, the policy seems to be changing on a day-to-day, hour-to-hour basis; and as always, we will continue to update you on developments about this issue as soon as information becomes available.
