DEA retirees on Medicare who did not receive 100% of their Medicare Part B premiums in 2018 or earlier will have to download the “Medicare Part B Reimbursement Differential Request Form” on the following link and mail it to the New York City Health Benefits Program, Attn: Medicare Part B Differential Unit (40 Rector Street, Third Floor, NYC, NY 10006) in order to get reimbursement. There are separate forms for 2016, 2017, and 2018. This form is only required for Medicare eligible retirees who did not receive the full amount of $134 per month. If you received $109 per month, the form will be required in order to get reimbursed the differential. The form and proof of the amount paid to Medicare will be required for reimbursement.
